No Physician-Patient Relationship

Accessing this website, completing a contact form, requesting an appointment, sending an email, calling the clinic, or communicating through social media does not establish a physician-patient or provider-patient relationship.

A professional healthcare relationship begins only after:

  • Aevum has accepted you as a patient;
  • Required forms and consents have been completed;
  • An appropriate clinical evaluation has occurred; and
  • A licensed Aevum healthcare provider has agreed to provide care.

Submitting an appointment request does not guarantee acceptance as a patient or approval for any treatment.

Eligibility for Treatment

All medical treatments, prescriptions, procedures, laboratory services, IV therapies, hormone therapies, weight-management programs, peptide programs, and wellness services are subject to clinical evaluation and approval.

Aevum may decline, delay, modify, or discontinue a requested service when a provider determines that:

  • The treatment is not medically appropriate;
  • Additional information or laboratory testing is needed;
  • Required monitoring has not been completed;
  • The treatment would create an unacceptable safety risk; or
  • Providing the service would conflict with applicable law or professional standards.

No Guarantee of Results

Results vary between individuals. Aevum does not guarantee any particular medical, wellness, weight-loss, hormonal, cosmetic, performance, anti-aging, or longevity outcome.

Testimonials, reviews, photographs, educational examples, and descriptions of patient experiences are not guarantees that another person will experience the same or similar results.

Prescription and Compounded Medications

Certain Aevum services may involve prescription medications, compounded medications, supplements, peptides, or off-label uses of medications.

References to any product or treatment on the website do not constitute a prescription or recommendation for you.

Compounded medications are prepared by compounding pharmacies for individually identified patients. They are not evaluated or approved by the United States Food and Drug Administration in the same manner as commercially manufactured FDA-approved medications.

Medication availability, pricing, formulation, shipping, and dispensing may depend on independent pharmacies and applicable law.

Appointments

Appointments may be requested through the website, by telephone, or through an independent scheduling platform.

An appointment is not confirmed until you receive confirmation from Aevum or its authorized scheduling platform.

You are responsible for:

  • Providing accurate contact and health information;
  • Completing requested forms;
  • Reviewing appointment confirmations;
  • Arriving on time; and
  • Informing Aevum promptly if you need to cancel or reschedule.

Cancellations, Late Arrivals, and No-Shows

Appointments are subject to the cancellation, rescheduling, late-arrival, and no-show policies provided during scheduling or contained in your patient financial agreement.

Late cancellations, missed appointments, or late arrivals may result in a fee, loss of a deposit, shortened appointment, or required rescheduling.

Pricing and Payment

Aevum is generally a direct-pay medical practice. Unless otherwise stated:

  • Payment is due at or before the time of service;
  • Prices may change;
  • Consultation fees do not automatically include laboratory testing, medications, supplements, pharmacy charges, procedures, supplies, shipping, or follow-up care;
  • Medication prices may vary by dose, pharmacy, formulation, and availability; and
  • Laboratory prices may vary depending on the tests ordered and laboratory used.

A posted price or estimate is not a guarantee of the final cost.

Aevum does not guarantee that an insurance company, health savings account, flexible spending account, or other benefit plan will reimburse any service.

Memberships and Recurring Payments

Aevum memberships may be subject to separate written membership terms, including:

  • The membership duration;
  • Monthly or annual charges;
  • Included benefits;
  • Excluded services;
  • Renewal terms;
  • Cancellation requirements;
  • Payment authorization; and
  • Treatment eligibility requirements.

By enrolling in a recurring membership, you authorize Aevum or its payment processor to charge the payment method on file according to the agreed schedule.

Membership benefits:

  • Are personal and generally nontransferable;
  • Have no cash value;
  • Are subject to scheduling availability;
  • Remain subject to clinical approval;
  • May not be combined with other discounts unless specifically permitted; and
  • Do not guarantee access to any prescription or treatment.

The terms of a signed membership agreement control if they conflict with this general website policy.

Refunds

Consultations, professional services already provided, completed procedures, laboratory orders, administrative fees, appointment deposits, compounded medications, prescription medications, supplements, and customized products may be nonrefundable to the extent permitted by law.

Medications generally cannot be returned after they have been dispensed or shipped.

Failure to achieve a desired result does not automatically create a right to a refund.

Any refund request will be considered under the written policy applicable to the particular purchase.

Mobile Services

Mobile services may be limited to designated service areas and may be subject to additional fees, minimum-purchase requirements, scheduling limitations, and clinical approval.

Aevum may decline or discontinue a mobile service when the proposed location is unsafe, unsanitary, inaccessible, disruptive, outside the service area, or otherwise unsuitable for medical care.

Electronic Communications

When you provide your telephone number or email address, you authorize Aevum and its service providers to contact you regarding:

  • Appointment scheduling;
  • Appointment reminders;
  • Forms;
  • Billing;
  • Patient-service information;
  • Administrative matters; and
  • Other communications you request or authorize.

Standard email and text messaging may not be fully secure. Do not send highly sensitive medical or financial information through an ordinary website form, standard email, or text message unless specifically instructed to do so.

Message and data rates may apply.

You may opt out of promotional emails or text messages by following the instructions in the communication. Opting out of marketing will not prevent appointment, billing, safety, or other necessary administrative messages.

Accuracy of Information

You agree to provide accurate, complete, and current information.

You must not:

  • Impersonate another person;
  • Provide false or misleading information;
  • Withhold material health information;
  • Submit another person’s information without authorization; or
  • Use another person’s payment method without permission.

Aevum is not responsible for harm resulting from materially inaccurate, incomplete, or withheld information.

These Terms are governed by the laws of the State of Texas.

Unless applicable law requires otherwise, disputes relating solely to use of this website will be brought in a court of competent jurisdiction in Harris County, Texas.

Aevum may revise these Terms periodically. Revised Terms will be posted on the website with an updated effective date.

Continued use of the website after revised Terms are posted constitutes acceptance of those revisions, except where additional consent is required by law.
